Dear [Recipient’s name],

I am writing to you today to formally request that my contract for phone service be cancelled on ___/___/_____ without any penalties or additional cancellation charges. This request is being made with regard to the Service members Civil Relief Act 50 U.S.C. 535(a) or the “SCRA” as legislated by the United States Congress and signed into law in December of 2003.

Reasons for Cancellation:

1. I am required to deploy outside of the continental United States for ninety days or longer.

2. I have received orders for a permanent change of duty station.

Due to the above listed orders, I no longer have the ability to satisfy the contract or to use the phone service.

The SCRA clearly states that upon receiving the request that the phone company shall grant the requested cancellation of the phone contract without issuing any early cancellation fees.
